Parish Verification Form

To be completed by new families or families who have moved parishes during the school year.

Through our parish verification process, families who are active in and contributing to any Catholic parish in the Diocese of Lubbock may request the registered and contributing parish discount towards tuition at Christ the King Schools. An active parishioner is defined as one who is registered at a parish within the Catholic Diocese of Lubbock, attends Mass faithfully, supports the life of your parish (ex: parent(s) serve as Eucharistic ministers, teach CCE, are on parish commission, etc.; child/ren is/are altar servers, tithes in a recordable manner to the parish, etc.). This form requires your parish priest’s signature and parish seal as well as a guardian’s signature. With this in mind, the completed form must be returned to the school office in order to receive the registered and contributing discount. Following each school year, the school will contact parishes to verify continued participation in order for families to continue to receive the special discount.
